Walk In Registration

Where: Mandt Community Center in Stoughton.

When: Saturday, September 11, 2010, 9:00 a.m. - 1:00 p.m.

Registration Forms: In order to save time, we recommend that you print and fill out the following forms prior to registration. Make checks payable to SYHA.

1. Registration Packet
2. Instruction Packet
3. Consent to Treat

To register by mail

1. Print and fill out the Registration Packet and Consent to Treat forms.
2. Mail completed forms with payment to SYHA Registration, Attn: Ryan Picek at P.O. Box 351, Stoughton, WI 53589.
3. Register your skater with USA Hockey's Online Registration.

Follow the registration instructions for the Learn to Skate (LTS) program. Please note that SYHA will accept late registrants for the Mini-Vikes and Mighty Vikes through December 31, 2010.
