Walk In Registration

Where: Mandt Community Center in Stoughton.

When: Saturday, September 13, 2008, 9:00 a.m. - 11:00 a.m.

Registration Forms: In order to save time, we recommend that you print and fill out the following forms prior to registration. Make checks payable to SYHA.

1. Registration Packet
2. Consent to Treat

To register by mail

1. Print and fill out the Registration Packet and Consent to Treat forms.
2. Mail completed forms with payment to SYHA Registration, Attn: Aaron Skinner at P.O. Box 351, Stoughton, WI 53589.

Follow the registration instructions for the Introduction to Hockey Program. Please note that you will be able to register your skater for the Ice Mites up until December 1, 2008.
