Parents registering competitive level skaters can qualify
for the early bird discount by registering in person on August 2 between
9:00 a.m. and 12:00 p.m. at the Mandt Center. To qualify for the early
bird discount via mail-in registration, your completed forms and full payment must be post
marked no later than August 2, 2008. Mail-in registration will be available via
the website until September 1, 2008. If you have questions regarding
registration please contact the ALD
for your skater's age level.
REQUIRED REGISTRATION PROCEDURES for 2008-09: USA Hockey will require that
all youth hockey associations convert to USA Hockey online registration.
SYHA will require all members to register for USA Hockey online. Please go to
http://www.usahockeyregistration.com
and follow the directions (detailed instructions)
to register your player(s), AS SOON AS POSSIBLE. The USA Hockey
registration fees can be deducted from your SYHA cash fees (see
registration packet for details).
There will be a
100.00 late fee assessed to all current SYHA skaters who do not register by
September 1, 2008*
To register by mail:
- If you are unsure of your carryover or credit from last year, please
contact Doris Johnson before mailing your registration.
She will respond with the confirmed amount. Chris Howell will work with you on getting
the $100 of raffle tickets to you that are required for each skater.

Consent to Treat and
USA Hockey Registration Receipt.
- Mail completed forms with payment to SYHA Registration at P.O. Box 351,
Stoughton, WI 53589. ALL forms must be received prior to try
outs. All Required Forms must be received prior to your skater participating
in tryouts.

*Skaters new to SYHA will be exempt from late
fees. Introduction to Hockey and Ice Mite skaters are also exempt from
late fees. All fees must be paid in full prior to your skater
participating in open hockey and tryouts. Please contact the

for your age level with any questions or concerns.
