REQUIRED REGISTRATION PROCEDURES for 2011-12: USA Hockey requires that
all youth hockey associations use the USA Hockey online registration system.
To be in compliance, SYHA must also require all members to register for
USA Hockey online. The total fee is $40.00 which covers both your
USA Hockey and WAHA registration fees. Note that skaters 6 and under are free,
but will still be required to register their skater.

Follow the site's online instructions. Be sure to print and retain
the confirmation(receipt) page after registering your skater(s) with USA Hockey.
You will need to submit this with your registration packet for the 2011-12 season.
ONLINE PAYMENT PROCESS for 2011-12: SYHA would like to have as many of our
members as possible use the online payment system. After registering your
skater(s) with USA Hockey use the following link to pay your SYHA cash fees.

Follow the online instructions and be sure to have your USA Hockey confirmation page.
You will need to enter the confirmation number during this process. There are two
questions that will be asked as part of the online registration process but
are not part of our registration. They are imbedded in the software
and we are unable to remove them:
- Volunteer Hrs Buyout - please answer 0.
- Prepay Puckbooks - please answer 0.
Once you've completed the online payment you will receive a confirmation
e-mail. You will need to submit a copy of this e-mail with your completed
registration packet (This applies to both mail in and walk in registration methods).
SYHA will no longer be accepting manual credit card payments.
If you intend to pay your fees using a credit card and opt not to use the
Online Payment Process. Then you will will need to use
PayPal to process your payment.
Steps for using PayPal:
- Login to your PayPal account and
click Send Money at the top of the page.
- Enter payment information and Continue.
- Click Change in the Payment Method
section and select the card that you intend to have
your cash fees charged.
- Review your payment and click Send Money. Payments should
be made to treasurer@stoughtonhockey.com.
- Retain you PayPal payment confirmation e-mail. You will need to send
this with your mail-in registration packet or provide a copy of this
e-mail during walk-in registration.
To Register By Mail:
- If you are unsure of your carryover or credit from last year, please
contact
Kristin Gibbons before completing and mailing your registration
packet. She will respond with the confirmed amount. You will be
able to pick up your $100 packet of raffle tickets from your ALD
during the week of tryouts.
- Print the
Registration Packet and
USA Hockey Registration Receipt.
- If you chose to use our
Online Payment Process, or
PayPal,
be sure to include a copy of the payment confirmation e-mail that you
should have received after completing the payment process.
- Mail completed forms with payment to SYHA Registration at P.O. Box 351,
Stoughton, WI 53589.
All Required Forms must be received prior to your skater participating
in tryouts.
Walk In Registration:
- Walk in registration will be held at the Mandt Community Center
on Saturday, September 10, 9:00 a.m. - 1:00 p.m.
- Again, If you are unsure of your carryover or credit from last year, please
contact
Kristin Gibbons prior to
the walk in registration date. She will respond with the confirmed amount.
You will be able to pick up your $100 packet of raffle tickets from your
ALD during the week of tryouts.
- To save time print and fill out the
Registration Packet form before coming to the Mandt Center.
A limited number of forms will be available at walk in registration.

Be sure to bring your
USA Hockey Registration Receipt with you to walk in registration.
SYHA will need your confirmation number for processing your registration
packet.
- If you chose to use our
Online Payment Process, or
PayPal,
be sure to include a copy of the payment confirmation e-mail that
you should have received after completing the payment process.
There will be a 100.00 late fee
assessed to all current SYHA skaters who do not register by
September 11, 2011*
*Skaters new to SYHA will be exempt from late
fees. Mini-Vikes and Super Vikes skaters are also exempt from
late fees. All fees must be paid in full prior to your skater
participating in open hockey and tryouts. Please contact the

for your age level with any questions or concerns.
