Thank you for your commitment to youth hockey in Stoughton, Wisconsin. Stoughton Youth Hockey Association will be paying officials on a monthly basis. After submitting a W-9 with our club, a check will be mailed to your address. If you earn more than $600 in a calendar year, a 1099 will be mailed to you for tax purposes. In order for you to get paid for officiating youth games at Mandt, we need you to complete a W-9 form. A link to this form appears at the bottom of this page.

The completed W-9 form can either be scanned and e-mailed to SYHA at syhapeewee@stoughtonhockey.com or the completed forms can be mailed to:
